Early Bird Rate (condition applies)

Please note for all those booking at the Standard Delegate Charge and confirming their place by Friday 14th October there is a special 'Early Bird' rate of £489 +VAT.

Standard Delegate Charge (£509 +VAT)

• I wish to attend the GFP Qualifieds Event on (Thursday & Friday) 3rd & 4th November 2011 which includes:
  
  
• 1. Access to both days and inclusive of lunch and all beverages during the Training Event.
• 2. Accommodation on Thursday 3rd November and breakfast on Friday 4th November.
• 3. A FREE reservation for the sponsored gala dinner and GFP Awards ceremony on Thursday 3rd November.
• I DO NOT wish to attend the conference dinner and entertainment on 3rd November.

Day Delegate Charge (£175 +VAT per day)

• I wish to attend the GFP Training Event as a Day Delegate which includes:
• 1. Access to either day (i.e. both days is £350+VAT) and inclusive of lunch and all beverages during the day(s).
• Thursday 3rd November
• Friday 4th November
• I will NOT be staying for lunch prior to departure on the 12th November.

• Please note that the additional charges below are outside the 'Standard Delegate Charge' detailed above and, are the responsibility of individual delegates booking directly with the venue hotel:
• £135 +VAT = Overnight accommodation on Wednesday 10th November and breakfast on Thursday 11th November.

• £35 +VAT = (cost of typical a la carte) dinner on Wednesday 10th November.

Payment Policy

Payment can only be made on receipt of invoice. This includes payment by procurement card. Payment can be made by cheque, payable order, procurement card or BACS. East of England Ambulance Service NHS Trust will invoice you from 20th October 2011.

Cancellation Policy

Any cancellation must be received in writing. Cancellations received up to 3 weeks before the event (by 12th October 2011) will receive a full refund less an administration charge of £100.00. Refunds cannot be made after this date.

Substitutions

Where a substitute cannot be found the full delegate fee will be charged. Terms will be strictly by return. All booking forms submitted (with or without all compulsory details) fall under our cancellation policy.
